Incentivizing the Outcome: Paying for Population Health at Hawaii FQHCs

AAPCHO’s Incentivizing the Outcome: Paying for Population Health at Hawaii Federally Qualified Health Centers project studies the impact of a team-level pay-for-performance (P4P) incentive program on health outcomes for high-risk AA & NHOPI populations at federally qualified health centers (FQHCs). Overall, the results of the study will contribute to reducing health disparities for these communities and provide a workable model to guide policy makers, health organizations, and health plans.
