Enabling services (ES) are non-clinical services such as health education and outreach that provide patients with healthcare delivery and access to care services and will play an increasingly important role under the Affordable Care Act. This presentation will describe the Association of Asian Pacific Islanders Community Health Organizations’ standardized model for collecting and analyzing enabling services data and its impact on health outcomes. In addition, the presentation will describe AAPCHO’s current effort, in collaboration Health Outreach Partners and National Health Care for the Homeless, in developing a training program to increase health centers’ capacity to implement the ES data collection model.
Session Learning Objectives:
- Understand the importance of collecting enabling services data provided by Community Health Workers and other ES workers using a standardized protocol;
- Familiarity with AAPCHO’s ES data collection model;
- Identify steps necessary to implementing the ES data collection model and its application at migrant and community health centers.
