Social Determinants of Health: Important Considerations for Medically Underserved Community Health Center Patients – Webinar Slides & Recording

Social determinants of health (SDOHs) are a wide array of social and economic factors, such as income inequality and unemployment, which have a tremendous impact on health outcomes. This webinar presentation provides an overview of SDOH and the role of SDOH in health care. The presenters also shared and discussed the experience and preliminary findings from studying SDOH at Asian Health Services, a federally qualified community health center. Finally, presenters demonstrated the use of community health center data within a software program that uses SDOH to adjust for patient risk.
