National Program to Eliminate Diabetes-Related Disparities in Vulnerable Populations

AAPCHO’s diabetes program aims to eliminate health disparities associated with diabetes by mobilizing and assisting local coalitions in their efforts to implement community-based interventions that reduce diabetes risk factors among AA&NHOPIs. AAPCHO’s coalition partners include CHCs, local businesses, community- and faith-based organizations, and government agencies that address diabetes-related disparities through policy, social and environmental changes.

GOAL
To reduce morbidity, premature mortality, and eliminate health disparities associated with diabetes by mobilizing community partners and assisting them in their plans to develop, implement, and evaluate community-based interventions that reduce the risk factors and influence the disproportionate burden of diabetes in AA&NHOPIs.
